 You may be aware that King Street Surgery will be merging with four other Hereford medical practices on the 1st July 2018, to become Hereford Medical Group.

Unfortunately, as a result of the merger process all Patient Access on-line services will be unavailable from the 1st May until the 2nd July 2018.

 

The following on-line Patient Access services will be unavailable from 1st May to 2nd July:

Ordering repeat prescriptions, booking appointments, sending electronic messages, changing address details and registering as a new patient.

 

Please phone our prescription line 9.30am-12.30pm Monday to Friday on the above numbers option 2, alternatively you can e-mail kingst.prescriptions@nhs.net or order in person at either site in writing.

To register as a new patient you must visit the surgery.

 

After the 2nd July 2018, patients who wish to use the Patient Access on-line services will be required to re-register for this service, by visiting reception at King Street Surgery or at another of the Hereford Medical Group sites with a form of ID.  We sincerely apologise for any inconvenience this may cause.  We would like to thank you for your understanding and patience during our merger process.


Practice Charter

 

We are happy to be members of a primary health care team dedicated to providing the best possible medical services for our patients.

 

This charter sets out the standards of services that our patients can reasonably expect. We continually monitor our performance against these standards.

 

Premises

 

We will provide and maintain clean, comfortable and accessible premises for the benefit of our patients.

 

Staff

 

Our staff will treat patients with due kindness and courtesy.

The duty of confidentiality extends to all members of our staff.

Patients can expect to speak privately to our doctors and all other members of our staff.

 

Appointments

 

Routine appointments will be available between 8.00am and 6.00pm Monday to Friday.

 

Patients will be seen within 20 minutes of their appointment time. If for any reason this is not possible, we will give an explanation for the delay.

 

Patients will be offered a consultation with a health professinal within 24 hours of their request or within 3 working days with the doctor of their choice whenever possible.

 

Telephone Consultations

 

Doctors will be available for telephone consultations at specific times between 8.00am and 6.00pm Monday to Friday, as stated in our practice leaflet and elsewhere on our website.

 

Home Visits

 

If for reasons of ill health or infirmity, patients are unable to attend the surgery we will visit them at home. We reserve the right to refuse a home visit and offer an appointment at the surgery the same day in other circumstances.

 

Requests for home visits should be made before 10.30am if possible.

 

Prescriptions

 

Requests for repeat prescriptions will be dealt with within 2 working days

 

Health Education

 

We will provide advice and literature to enable our patients to take responsibility for their own health.

 

Copies of Patient Notes

 

We are more than happy to provide copies of patient notes however the request needs to be made in writing to the Practice Manager Mrs Katie Jones. Please allow 7-10 days for this to be completed. Copies of notes once complete will need to be collected from either site.   A leaflet explaining this service is available from reception.
